Health Declaration

I confirm that:

• I am voluntarily participating in exercise activities.

• To the best of my knowledge, I am medically fit to exercise.

• I have consulted my GP or another suitably qualified healthcare professional if I have any medical concerns.

• I will inform my coach of any injuries, medical conditions, pregnancy, or changes to my health that may affect my participation.

Understanding the Risks

I understand that physical exercise carries inherent risks including but not limited to:

• Muscle soreness

• Sprains and strains

• Joint injuries

• Falls

• Cardiovascular complications

• Aggravation of existing conditions

• Serious injury or, in rare cases, death

I freely accept these risks.

My Responsibilities

I agree that I will:

• Exercise within my own limits.

• Use appropriate equipment safely.

• Stop exercising if I feel pain, dizziness, chest pain or any unusual symptoms.

• Seek appropriate medical advice where necessary.

• Follow coaching instructions responsibly while recognising that I remain responsible for my own safety.

Coaching Disclaimer

I understand that coaching is educational and motivational in nature.

My coach cannot guarantee results and is not providing medical diagnosis, treatment, physiotherapy, psychological counselling or other regulated healthcare services.

Results

I understand that fitness outcomes differ between individuals and depend on many factors beyond the coach’s control.

No guarantees have been made regarding weight loss, body composition, strength, athletic performance or health improvements.

Release of Liability

To the fullest extent permitted by law, I accept responsibility for my decision to participate and acknowledge that exercise involves inherent risks.

Nothing in this agreement excludes liability for death or personal injury caused by negligence, fraud, or any other liability that cannot legally be excluded under UK law.

Photography and Testimonials (Optional)

If I voluntarily submit photographs, videos or testimonials, I consent to their use for marketing purposes unless I withdraw that consent in writing.

Electronic Agreement

By ticking the box stating “I have read and agree to the Terms and Conditions and Informed Consent and Privacy Policy”, purchasing a programme, booking coaching or accessing digital content, I confirm that:
